Referral Information for Inpatient and Outpatient Programs
Referrals to Hickory Trail Hospital and Branches Dallas programs can be made by anyone – including healthcare professionals and providers, case managers, other community professionals, or family members. When outpatient providers are involved in a patient’s care, it’s important to establish intent and provider relationship at the start of the admission process to ensure the best possible outcomes.
To streamline the referral and support effective discharge planning, please complete the referral form and fax it securely to the Intake Department at (214)-389-7444 or [email protected].
Please note: submitting a referral form does not guarantee program availability, but it helps our admissions team prepare for a smooth and efficient process.
